Acupuncture, moxibustion, massage, and shiatsu are covered by health insurance benefits (for medical care expenses) if certain conditions are met.

Required documents:	Application Form for Medical Care Expenses (acupuncture, moxibustion) Application Form for Medical Care Expenses (massage, shiatsu)
	[Documents to attach] Letter of approval from an insurance doctor Receipt (original) Report on treatment (copy) (when obtaining subsequent approval)
Deadline:	As soon as possible
Applies to:	Insured persons and dependents meeting the conditions for payment of medical care expenses and have been examined and received letters of approval from insurance doctors
Address inquiries and Submit to:	The person in charge of social insurance administration at your company (If you are voluntarily and continuously insured, please inquire with the Health Insurance Society instead.)
Notes:	If continuing treatment is deemed necessary after six months (one month for manual correction of deformities), a second letter of approval from an insurance doctor must be obtained.
