After you leave your employer

After leaving your employer, you will lose your eligibility for membership in the Health Insurance Society and must join the appropriate medical care insurance program based on your individual circumstances.

Return your health insurance card when you leave your employer

Required documents:
  • Health insurance cards (for the insured person and all dependents)
  • Elderly benefits card (if issued)
Deadline: Within five days after the date of loss of eligibility
Applies to: Insured persons leaving employment and their dependents
Address inquiries and Submit to: The person in charge of social insurance administration at your company (If you are voluntarily and continuously insured, please inquire with the Health Insurance Society instead.)

If you wish to remain a member of the Health Insurance Society

Required documents:

  • Direct Debit Authorization Form(If you chose to pay insurance premiums every month)
    • ※If you are unable to submit your application form in advance, we will send you an application form after enrolling.
Deadline: Within 20 days after the date of loss of eligibility of the insured person
  • ※Note that if the application form is not submitted by the deadline, enrollment will not be possible.
Applies to: Insured persons who have been insured continuously for at least two months prior to leaving employment
Address inquiries and Submit to: Health Insurance Society or The person in charge of social insurance administration at your company (If you are voluntarily and continuously insured, please inquire with the Health Insurance Society instead.)

Caution:

  • If you have a medical consultation at a medical institution before your voluntary and continuous insurance card arrives, please tell the medical institution that you are still carrying out the voluntary and continuous insurance procedure and therefore do not have an insurance card.
  • * If you have a medical consultation at a medical institution without an insurance card, please follow the medical institution's instructions.
